Enter your login credentials

Click “Barangay Information System”

Click the dropdown arrow beside “Official’s Profile” under
the Menu Bar and select “Officials”
Step 5.1

Enter the name of the DECEASED BARANGAY


OFFICIAL in the search box
Click Advance Search (if the name of the deceased
barangay official does not show through the search
box)
Fill in the information of the deceased barangay
official and then click search

If the name appears in the SEARCH BOX there is
no need to use the ADVANCE SEARCH.

If the name appears, Click the name of the


DECEASED BARANGAY OFFICIAL

Click the EDIT button to update

If the barangay official has NO RECORD in the


BIS. Their information must first be encoded.

Click “CREATE
PROFILE” to encode the information of the
barangay official

If the barangay official ALREADY HAS RECORD


in the BIS, this step should be skipped.

indicated more than one beneficiary

Set the status to DECEASED

2. Cardiac Arrest
3. Heart Disease
4. Stroke
5. Cancer
6. Pneumonia
7. Diabetes
8. Respiratory Disease (TB, COPD, etc)
9. Kidney Disease
10. Assault, Death under Investigation
11. Death during police Anti-Illegal Drug Operations
12. All other causes

Click “UPDATE” after reviewing all the data encoded and documents were uploaded

Click the dropdown arrow beside “Death
Benefits Claim” under the Menu Bar
and select “Officials”

Enter the name of the DECEASED BARANGAY OFFICIAL in the search box and hit “ENTER”

Click “PROCESS CLAIMS”


After clicking “PROCESS CLAIMS,” you will be directed to this page
Step 10

If the DECEASED BARANGAY OFFICIAL


has more than one beneficiary specified
in the BOIS, choose among the names
that appear in the drop down as to who
will be the claimant

Indicate in the “REMARKS” the documents
submitted by the CLAIMANT and upload
the said documents into the system
Step 10

Upload Application Form for DBC, Death Certificate and other evidentiary
documents in separate PDF Files (single file document is not allowed). Total file size
of uploaded documents should be 2MB or less. Otherwise, you may have your
documents compressed thru <https://www.ilovepdf.com/compress_pdf>

Click the “UPLOAD” button.

Do not click the individual UPLOAD Icon for


each file.

Then, Click the “HOME”
button.

You will be directed to this page as soon as all the


documents were successfully uploaded

This is the DASHBOARD which displays all the STATUS of
the DEATH BENEFIT CLAIMS processed.

Click “CLAIMS TO PROCESS” to


initiate submission to the
Provincial Office
This is the PROCESS CLAIMS page which displays all
processed Death Benefit Claim in a particular LGU for
submission to the Provincial Office.

Click “CHECK BOX” of the Deceased Barangay


Official/s for submission to the Provincial Office

Attach the Transmittal Letter

Click the “UPLOAD” button.

Do not click the individual UPLOAD Icon


Step 14

This page displays when the


submission to the Provincial Office
is successful.
