What is a PTW System :
A PTW system is a formal written system used to:

• Control certain types of work which are identified as potentially hazardous.

• Means of communication between site/installation management, plant supervisors and

operators and those who carry out the work.

• Sets out the main precautions needed to complete the job safely.

When are PTW Systems Applicable :

Permits to Work should be considered whenever it is intended to carry out any work which

may adversely affect the safety of personnel, the environment or the facilities.
Responsibilities:
• Area Supervisor: Is the person who has overall responsibility for the co-ordination of work, for
authorizing all Permits in his area of responsibility.

• Area Authority: The Operation’s representative with operational responsibility for a specified Area.
He is directly responsible for the control of work done in that area at any particular time. He is the
person responsible for the Validation and Re-validation of Permits each shift, and finally for
Cancelling (closing) Permits.

• Permit Applicant: The person who applies for a Permit and completes the Work Description parts of
the Permit, for approval by the Area Supervisor. He is responsible for ensuring that a Permit Holder is
nominated at all times when the work is under way.

• Permit Holder: Is the person nominated by the Permit Applicant to be in charge of a worksite. He is
responsible for ensuring that the work party obeys the requirements of the Permit to Work System
and other relevant procedures.
Responsibilities:

Authorized Gas Tester (AGT) :


The Authorized Gas Tester must be trained and assessed as competent, authorized by the
Installation/Site Manager and registered to test for the presence of flammable vapors, toxic gas, and
oxygen as per the Permit’s precautions/instructions.

Isolating Authority (IA) :


The Isolating Authority is responsible for isolating specific sections of plant or items of equipment to
the highest integrity, which is reasonably practicable. The IA is also responsible for demonstrating the
integrity of the isolation to the Area Authority and Permit Holder and for monitoring the integrity of
isolations when in force. The IA can also be the Permit holder if required. For electrical isolations, an
Electrical in charge is to be used be used (refer to the local Electrical Safety Rules).
When to Use PTW:
• Hot work of any type where heat is generated or used.
• Work which may cause an unintended or uncontrolled hydrocarbon release, including any
disconnection or opening of any closed pipeline, vessel or equipment containing, or which has
contained flammable or toxic material.
• Electrical work which may cause danger.
• Entry into confined space and or work inside them.
• Work involving the use of dangerous substances, including radioactive materials and explosives.
• Excavations.
• Pressure /leak tasting.
• Simultaneous operations.
• Work that is an unusual nature or is new to site.
• Maintenance work which compromise critical safety systems or remove them from service e.g. fire
and gas detection system etc.
• All work carried out at any of the Haya water sites or facilities by contractor.
• Non-routine work carried out by operation & maintenance team member.
Exception from using PTW:

• No isolation of hazardous energy source is required.


• No LOTO is required.
• No specialized PPE or safeguards required.
• Work is of routine nature and a known repetitive task and covered by the operating procedures.
• Work activities inside the office building such as cleaning, pantry area, photocopying etc.
• Work/activities inside workshops and other non-hazardous modules .
• Operating valves or applying LOTO as part of equipment preparation.
• Processes involving naked flames or hazardous substances in approved laboratories
• Operation of equipment for approved training purposes e.g. use of firefighting during drills
• Work of a cold nature (i.e. machining, fitting, turning, and calibration work) in workshops in Non-
Hazardous areas.

Job Safety Analysis:

It is mandatory to carry out Job safety analysis (FOM-HSE-077) (Hazard Checklist )for the following work.

• Hot work
• Confined space entry work
• Excavation
• Work at height

Job safety analysis can be done if the Area authority /Permit holder/ Area supervisor feels the risk reduction is necessary.

Display of Permits and Certificates:

• The original permit must be displayed at the location of the work.


• The permit duplicate must be displayed on the Permit Status Board.

‘Outstanding’ section of the Permit Status Board:

• Permits raised for long term isolations.


• Permits for work that was not completed. (in progress)
• Permits for work where equipment has not been de-isolated.
• Permits raised for isolations.
Emergency Work Situation:

An emergency is the only time when work can proceed prior to a permit being issued. In an emergency, work can proceed
as directed by the Area Supervisor. Permit issue shall follow as soon as practical after the emergency.
Emergency situations include Fire, Major Gas Release, Major catastrophic conditions, etc. (as defined in emergency plan)

Response to emergency:

If any emergency alarm sounds, all works must be stopped, all permits issued shall be withdrawn ,cancelled (closed)
during emergency.

Filing of Permits:

After sign off by the Area Authority, the permits and isolation certificates must be filed all copies of the
permit after completion should be returned to Area Supervisor for filing.
