BACKGROUND/MOTIVATION

According to the study, police investigator when


writing police or investigation reports or documents, it
finds very difficult to express their thoughts and the
facts of the case clearly, whereby his or her prepared
reports or documents become incomprehensible or the
meaning of their contents is vague, resulting in the
unfortunate dismissal of some cases they handled.

BASIC TYPES OF INVESTIGATIVE


REPORTS
1. Police Blotter
• is a record or log of daily events occurring
within the territories/jurisdiction of a given
police unit or command.

The following incidents or transactions, among others, are entered in the Police
Blotter:
a) Violations of laws, ordinances reported and or discovered;
b) All calls in which any member of the PNP is dispatched and/or takes official action:
c) All fire alarms, reports and information received by the stations;
d) Movement of prisoners with corresponding notations on the authority for such
movement;
e) Cases of missing or found persons, animals and properties;
f) Vehicular and other types of accident which require police action;
g) All personal injuries, found bodies, and suicides;
h) Damage to property;
i) All cases in which a police member is involved;
j) All arrest and return made; and
k) Miscellaneous cases, general and special orders, violation of rules and regulations
and any other reported incident that the Sub-Station/Station Commander or higher
authority desires to be recorded.

ted in the arrest of Ronnie Maboloc y Ramirez, 25 years old, construction worker, resident of Brgy. 104, Salvacion,
Tacloban City and a drug watch listed personality. Bought from the suspect was one (1) piece heat sealed trans-
parent plastic sachet containing white crystalline substance suspected to be “shabu” worth five thousand pesos.
Subsequently, a search incidental to a lawful arrest was made on the suspect which resulted in the recovery of: five
(5) sachets containing white crystalline substance suspected to be “shabu”; five thousand pesos boodle money
having one (1) piece subscribed Five Hundred Peso (Php 500.00) with serial number MK519916 used as buy-bust
money and one (1) unit Samsung cellular phone. The search, markings and inventory of the subject of sale and the
recovered pieces of evidence were done at the place of arrest and in the presence of witnesses from themedia and
elected Barangay Official, in the person of Mr. Eul Caorte and Hon. Cleto Baldo, respectively. The recovered items
have a total estimated market value of fifty-five thousand pesos (Php 55, 000). On scene inspection was conducted
by PMAJ DANIEL V POLO, Station 2 Commander. The Poseur Buyer, Seizing Officer and Evidence Custodian of
the said operation was PSSg Archibald C Balais while the Arresting Officer was PCpl Jordan M Daa. Criminal
cases for violation of Section 5 and 11, Article II of R.A. 9165 against the suspect, are now being prepared to be

What: Flash Alarm re Missing Person
When: 10:00 AM of June 13, 2020 (Time of Incident)
Where: At the seawaters of Limasawa, Southern Leyte.
Who: Jemuel D. Tio, 35 years old, widower and a resident of Brgy.
Lugsongan Limasawa, Southern Leyte.
How: Limasawa MPS received a report that the above named person was
last seen sailing in the seawaters of Limasawa, Southern Leyte for fishing.
He was with other fishermen who were on board with another pump-boat.
When one of the pump-boat was already full with flying fish (Bangsi), the
other fishermen intended to transfer to the pump-boat of Jemuel so that
their catch will be brought to Padre Burgos shores. They intended to
transfer to the boat of Jemuel, by way of swimming, however, due to the
strong current, the pump-boat of Jemuel sank. While struggling to swim
another pump-boat passed by, helping the other fishermen to get into the
boat. Jemuel on the other hand was lost in sight due to bad weather and
strong current. Search and Rescue operations were already conducted by
Limasawa MPS personnel together with Philippine Coastguard Limasawa
Sub-station and the MDRRMO. For information.
