• Marafiq Yanbu 2 project achieved 296 working days with a total of 18,056,436 man-
hours without Lost Time Incident.

• SADARA PE project achieved 7,141,255 safe man-hours since last recordable


(MTC) and 9,404,831 man-hours without Lost Time Incident (LTI).

• SADARA Chlorine project achieved 4,301,315 Man-hours without Lost Time Incident
(LTI).

• STWP project (Y1) achieved 1,065 working days with a total of 841,720 man-hours
without Lost Time Incident (LTI).

• LUBEREF project achieved 311 working days with a total of 760,403 man-hours
without Lost Time Incident (LTI).

• Fuel Supply project (Y1) achieved 879 working days with a total of 1.7 million man-

Jacobs Off the Job MVI


OVI #1 – On 04 Dec 2013, a Jacobs vehicle parked (unoccupied) in front on a shop in Dhahran
road was hit on its rear by a third-party vehicle causing damages to its rear bumper and boot
lid. No injuries involved. As per the employee, the third party vehicle was attempting a drift feat
on the road which went out of control resulting in the incident.

Controlled Contractor Incidents

First Aid Case (FAC) #1 :


30 November 2013, In Sadara Chlorine site: A Controlled Contractor the air duct erector was
transferring duct materials in one place to another, during the operation the air duct erector is
closely interact with the corner of scaffolding pipe which is caused a minor scratch on his right
eyebrow. The worker was immediately taken to the site First Aid Station and he were given a
first aid treatment.

First Aid Case (FAC) #2 :


5th December 2013, New Office Building site: A contract worker from Al Khobar New Office
building project suffered minor contusion injury on his left leg when a box of material
unexpectedly tilted against him, pinning his lower leg and resulting in the injury. He was
immediately taken to a nearby hospital for medical examination and returned to work after
receiving first aid treatment.

03 December 2013, In Marafiq Yanbu 2 site: A Non Controlled Contractor excavator was
assigned to remove soil for construction of storm water culverts, during excavation the
operator accidentally hit and damaged the extension cables that was buried in the ground
which provide supply in Electro Chlorination Plant.
The site electrician was informed immediately & he disconnect and remove the damage
cable. No injuries involved.
Incident and investigation is ongoing.

Site HSE Activities


The Turn-Around is now mostly completed, all remain is some in insulation works. Turn-
Around tie-ins activities is being monitored, 82% completed (78 of 121 tie-ins) are now
handover to Luberef while all electrical tie-ins are completed for SS 1, 4, 8 & 9. Samsung

HSE Concerns
Project expected to reach 1,000,000 man-hours on first week of January 2013, Everyone is
alerted to stay focused and continued monitor the site activities

Marafiq Yanbu1 - SWTP


Site HSE Activities
• Restarted construction works and some of the hazards & risks was encountered with civil
works.
• Medical evacuation drill has been conducted on 05 December 2013, its took 13 minutes to
take out the victim via tower crane in the rescue basket.
• Good dust control all over the site and maintaining good practices of housekeeping and
good PPE compliance.
• On-site Safety training provided for the rigging crew.
• Nurse for night shift has been provided while dayshift nurse has extended their time for
back-up.

Marafiq Yanbu1 – IWTP II


Site HSE Activities
• Daily basis Coordination/Commissioning Meeting for any safety concerns with PMC
commissioning team and daily inspection of PTWC and LOTO implementation.
• Mass tool box done on “Hearing Protection & Mandatory PPE” & “Spill/Leak at Sludge
Dewatering Incident (Lessons Learned)”.
• Conduct a Rescue drill at Chlorination bldg. and safety inspection of chemical storage
areas.
• Successfully completed diving operation at Nitrification Basin-A.
• Zero tolerance for non-compliance of fall protection, electrical and confined space
activity.
• Witnessed the training of Chemical Handling conducted for contractor’s operator.

Site HSE Activities
• Continuous disposal of ash waste at ESP 5 was done safely by environmentally
accredited disposing company.
• Inspection on STG 5 and 6 lay down yard, re-arrangement of remaining materials and
disposal of unwanted materials was done safely.
• PTWC and LOTO implementation has been improved.
• Smooth synchronization of UNIT 6 and ongoing installation of GRP pipe (2.4m dia.) at tie-
in#3 was done safely.
• EPC provide special training for confined space entry for shutdown activity and faults on
Fire Alarm System were detected and fixed safely.
• Special training has been conducted for Monitoring of shutdown activities at confined
space of STG 5 and Boiler 5 units and monitor of ongoing excavation and GRP pipe (2.4
dia.) laying at tie-in#3.

Marafiq Yanbu1 – Small Projects


Site HSE Activities
• Management safety walkthrough on Potable Water Tank, Fire Water LT&D, replacement of
asbestos sheets at PSIP & PSDP and Lift Station Projects have been conducted.
• Disinfection of all lines of new potable water tank has been completed safely.
• Removal of asbestos sheets at PSSP & PSIP buildings in SWTP.
• Review HSE Plan & Risk Assessments and interview of Safety Officers for upcoming new
projects.
HSE Concerns
Replacing of asbestos sheet at PSIP, PSSP & PSDP and lift station projects.

Marafiq Yanbu1 – Fuel Supply


Site HSE Activities
• Modification of Foam system and testing of fire hose cabinet has been completed safely
and still waiting for the first drop of HFO fuel.
• Monitoring Working at height for heat tracing is ongoing and installation of lightings system
at Fire Foam System is completed.
• LOTO applications is being monitored and preparation for check in of first drop fuel.
• is reviewing the unsecured grating platforms and their action taken is to fixed/secure all
grating platforms.
HSE Concerns
LOTO application for some balance on electrical activities prior to first drop of fuel HFO.

Marafiq Yanbu2 Projects

Site HSE Activities


• MET had been conducted a random inspection of scaffolds in the pipe racks.
• TSA & PTW issues are still being strictly implemented in Package #1 & #3. Several works
were suspended for the rectification of these issues, including sending the foremen,
supervisors & even HSE personnel of subcontractors to refreshers training.

Marafiq RO4 - Jubail


A total of 775 personnel have undergone through safety orientation to date, newly mobilized
workers of 710 and 65 visitors/subcontractors
• An average of 465 manpower on site and its being monitored by 9 Safety Personnel with 2
Saudi Trainee having a ratio of 51:1.

Site HSE Activities


• Mass Toolbox talk was held on 23 November 2013. Topic “Scaffolding Inspection” it was
given in English & Urdu and PMC-HSE advised the worker regarding safe working.
• Safety Spot Quiz Program: Prepaid cards distributed to those employees who replied to the
question asked.
• In house training for Safe Unsafe Act (SUSA) has been conducted by PCMC as per
requirement of PMC-HSE.
• Encroachment Permit from Royal Commission for 4 Kilometer cable laying activity was
already approved and released.

HSE Concerns
• Safety Campaign for “Chemical Handling Safety” and monitoring of civil activities.
• Start mobilization/site preparation for 4km cable outside activity and assigned Safety
Officer.

Marafiq EPS – Jubail


Site HSE Activities
• Weekly Joint Site Safety Walkthrough is in Progress training for Critical Risk Assessment
for Hazardous Materials and Substances including safety requirements in FRP lamination
has been conducted by PMC-HSE.
• Risk Assessment have been submitted and reviewed by PMC for Core Drilling of existing
Pump House wall for electrical cable routing to MASA and Tie-In 011/012 and Assembly
and Disassembly of Crawler Crane.

HSE Concerns
Provide additional life support at Switchgear and inspection of Third party SETE Excavator
for renewal and installation of access scaffold for electrical pull pits.

CPChem – Jubail
Site HSE Activities
• Hand Safety Awareness Campaign has been raised on 30 th November 2013. This campaign
set was continue for a minimum of 4 weeks to focus the PCC project on hand safety with
awareness talks, posters and signs to remind employees of hand safety and protection
methods.
• Develops Supervisor Orientation, Inspections of equipment and review new Jacobs HSEPs.
HSE Concerns
• They have significant observation on site were identified employees are not using 100% fall
protection, unstable of scaffold platforms and electrical cables laying on a wet surface.
• Safety issues on site are fall protection and specific tasks of SPA is not completed.

• OSHA 30hours (classroom training) second session was conducted on 30 November 2013.
• Beyond Injury Free (BIF) Training session has been conducted by HSE Department for
staffs, next session to be scheduled and Jacobs Orientation for newly arrive at site.

• Weekly HSE Walkthrough was organized & workers were rewarded as a part of ongoing
Safety Recognition program.
• Beyond Injury Free Training done at site was attended by 34 project team members.

SOR Al Khobar Office :

In Jacobs ZATE home office ground floor, the floor is very slippery, even when
dry. This might be due to very smooth surface of the tiles and/or too much
polish used.

Corrective Action Taken


Verify the cleaning and polishing procedure.

SOR Al Khobar Office :

Several of our doors from the elevator lobbies into our secure areas are in need
of maintenance so they will close well and lock securely. This is an office
security issue.

Corrective Action Taken


Checked the doors and informed facilities regarding the faulty ones. Access
control contractor was informed of the problem and we are waiting for the
access control contractor to address the issue.

SOR Al Khobar Office :

Found an empty toner cartridge replaced from machine by unauthorized person


near in trash bin and not properly discarded. The ink was spilled in the floor and
trash bin cover.

Corrective Action Taken

Immediately removed the empty cartridge and informed facilities to clean the
scattered ink in the floor and trash bin cover.
Notice was placed near the printer warning employees no to attempt unauthorized
maintenance.
