Introductio
n
Tracheostomy is a medical
procedure that helps people
breathe easier. It involves
creating a small opening in
the neck and inserting a tube
into the windpipe (trachea).
Definition
Tracheostomy is a surgical
procedure creating an
opening in the trachea to
facilitate breathing .
Purpose
The primary purpose of
tracheostomy is to provide a secure
and stable airway for patients with
respiratory distress or airway
obstruction,enabling them breathe
comfortably and efficiently .
Indication
 Respiratory failure
 COPD
 Pneumonia
 ARDS
 Trauma of the neck or
throat
 Tumors or cyst in the
Cont......
 Foreign body obstruction
 Severe head or neck trauma
 Burns to the face or neck
 Prolonged Mechanical
ventilation
 Chronic
respiratory insufficiency

1. Surgical Tracheostomy :-
 This is an open surgical procedure
performed in the surgical room . An
incision is made in the trachea and
a tube is inserted. It is often used
for long term respiratory support .
2. Percutaneous
Tracheostomy:-
 This is a less invasive procedure often
performed at the bedside, commonly
in intensive care units . A needle is
used to puncture the trachea,then
dilators are used to insert the
tracheostomy tube. It's typically for
patients needing medium - term
airway access.
3. Temporary

Tracheostomy:-
Performed when a patient requires
short- term respiratory support,such as
during certain surgery or recovery from
trauma ,with the expectation of
eventual removal of the tube .
4. Permanent

Tracheostomy:-
This is usually indicated for patients who
will need life - long respiratory support
due to condition like advance cancer or
neurological diseases . The
tracheostomy opening is intended to be
a long term solution .
Tracheostomy tube
types
I. Cuffed tracheostomy tube - prevents
air leaks and aspiration.
II. Uncuffed tracheostomy tube - used for
patients without aspirations risks .
III. Fenestrated tracheostomy tube -
allowing for speech and swallowing .
IV. Double lumen tracheostomy tube -
inner and outer cannulas for easy
cleaning .
Equipment :-
 Tracheostomy tube
 Tracheostomy tube ties
 Neck plate
 Suction equipment
 Humidification device
 PPE
 Other supplies like gauze, hydrogen
peroxide, normal saline sterile water,
scissors and sterile cotton swabs .
Procedure of
tracheostomy
1. Preparation
 Consent and preparation
 Positioning
 Sterilization
2. Anesthesia
3. Incision
4. Creating the stomach
5. Insertion of tracheostomy
tube
Complications
 Bleeding
 Infection
 Tracheal stenosis
 Tube displacement
 Blockage of tube
 Aspiration risk
Nursing Management
 Before the tracheostomy procedure, explain the
purpose of the procedure to both the patient and
care giver.
 If the patient is having tracheostomy in the
operating room, follow all hospital policies and
procedures for preparing the patient for surgery .
 If the procedure is prepared in the patient's bedside
ensure all appropriate personnel including
respiratory therapy .
 Record vital signs including heart rate , respiratory
rate and rhythm ,bp and spo2 .
Cont.....
 Set up all necessary equipment .
 Position the patient supine .
 The nurse educate the patient and his
family about transmission of infection
after discharge .
 Assess for symptoms of infection .
 Patient education like -
 Daily cleaning and suctionig techniques .
 Emegency actions for tube blockage .
 Long term monitoring and tube changes .
Conclusion
A tracheostomy is a surgical procedure
that creates a hole in the neck and into
the windpipe to help deliver oxygen to
the lungs. The procedure can be
performed in number of ways including
open or percutaneous techniques. The
gaol of tracheostomy is to provide a
stable airway and
improve patient comfort .
