What is a Work Permit System?
☞ Permit to work is a formal written system used to control certain
types of work which are potentially hazardous.

☞ It provides guidance to the workforce to identify any possible


hazards and to undertake the necessary precautions before
commencing such work.

☞ Permit to work system addresses the work to be completed, the


equipment to be used, how to prepare the location, safety
precautions and any procedures that must be followed for safety.
Permit to Work System: When?
PTW systems shall be considered for following but not
limited to:
•Non-production work (e.g., maintenance, repair,
inspection, testing, alteration, construction, dismantling,
adaptation, modification, cleaning etc.);
•Non-routine operations;
•Jobs where two or more individuals or groups need to co-
ordinate activities to complete the job safely; and Jobs
where there is a transfer of work and responsibilities from
one group to another.
Permit to Work System:
Exclusion List
All activities shall necessarily need to have work permit unless and otherwise stated
in exclusion list which is approved by unit apex committee.
Exclusion list may include:
•Routine Jobs which are necessary part of manufacturing process for that area.
Eg. Mining, Loading, Transportation, Unloading, Stacking, Reclaiming, coal
feeding, clinker handling, cement Grinding packing till loading of bags
•Apart from above those jobs where energy source isolation is not required, where
special work permit is not required and where potential hazards are identified & risk
mitigation plan is available in the form of SOP.
•Quality control Job like collection of sample, raw material handling etc./
•Routine activity not requiring special permit in General Stores.
•Cooking, inspection, Audits, Training and other Jobs of administration in nature.

Each department shall prepare exclusion list of activities where general work permit
is not required and where hazards are identified and get it approved from Unit Apex
EHS committee.
Integrated Work Permit System
• “Integrated Work Permit” is generally implemented for
UltraTech Cement Limited-Green and Brown field Project sites
only.
• This integrated work permit covers all high risk activity except
blasting.
• It sets directions for assigning accountability to site engineers
and contractors partners for ensuring safe practices and
performance
• This system will in principle cover all on-site routine and non-
routine work which may create potential hazards.
• All Project site activities shall necessarily need to have work
permit unless and otherwise it is stated and approved by Project
Head.
Integrated Work Permit System
Activities included in UTCL’s Integrated Work Permit are:
1)Hot Work (Welding, Cutting, Grinding)
2)Excavation exceeding 1 meter (3 feet)
3)Work at Height (More than 1.8m height)
4)Suspended Man Basket/Platforms
5)Crane lifts & Critical Rigging
6)Electrical Work
7)Scaffolding Erection & Dismantling
8)Hydro/Pneumatic Testing
9)Confined Space
Definitions
Custodian (Area In charge)
Custodian means any on-site company’s senior employee, who is a
designated by Project Safety Committee, as owner of clearly marked
area on plot plan having all facility, building, structure, or any
equipment on-site within the functional area.

Issuer
Issuer means company employee designated by custodian with
relevant job experience & authorized by Project Safety committee who
will issue work permit for respective area. Issuer should be
knowledgeable of the hazards and safety requirements for the
equipment and process, and who is trained in Work Permit processes
and shall be responsible to get physical isolation of all hazardous
energy sources to provide safe work environment to initiator. It is
recommended that issuer should have 5 years work experience.
Definitions
Initiator / Requestor
Initiator means any person, from the job executing agency /
Contractor, having relevant job experience & authorized by Project
safety committee. It is recommended that initiator should have 5
years work experience.

Authorized isolator
Authorized isolator means the person identified and authorized by
Unit Apex Committee. Authorized isolator shall be different for
different discipline like Electrical, Instrumentation, Mechanical and
Process. It is recommended that Authorized isolator should have 3
years work experience in his functional area.
Definitions
Co-permittee (Permit Taken By)
Co-permittee means contractor supervisor or the gang leader of work group
who is carrying out the job at site. May be Sub-contractor also, having
relevant job experience & authorized by Initiator. It is recommended that
Co-initiator should have 5 years work experience.

Hazardous Energy Sources


Hazardous Energy Sources Any source of electrical, mechanical, hydraulic,
pneumatic, chemical, thermal, nuclear radiation or any other energy that, if
not controlled, could cause injury to personnel or damage to property
and/or environment

Major Safety Equipment


An equipment, processes and systems specifically designed to protect the
safety and health of personnel and the environment.
Responsibilities
The Project Safety committee shall ensure that the permit-to-work system is:
•Functioning and maintained in accordance with its objectives.
•Audited, reviewed & revised as per management systems.

Line managers are responsible to ensure that:


•All their staff and contractors under their control understand the system.
•Adequate training and instructions are given to their staff and contractors.

Custodian shall be responsible for issuing the permit to work in their respective
areas.

Initiator shall be responsible to obtain permit for the job, he is executing. He


needs to take integrated work permit for one location, level, type of work &
contract agency.
Responsibilities
All individuals of the work group working within the site have the
responsibility to
•Have a good understanding of the permit-to-work systems and they have to
follow the SOP/SWP briefed them before commencing the job.
•Ensure that they do not start work on any job requiring a work permit until one
has been authorized and issued;
•Follow all the precautions and safety measures stipulated in the permits and
strictly comply with the requirements and procedures of the permit-to-work
system;
•Get advice immediately from initiator if they are in doubt or if any circumstances
or change of conditions make the work area unsafe.
•They should not adopt any short cut while performing the job.

1) Permit to be signed by people not less than Site Engineer (from ABG
/UTCL/Contractor’s) .
2) Permit requester may be party who raised work order to frontline working
agency, issuer may be party owning site and receiver may be the working
agency or agency who had awarded work order for the specific job.
3) This work permit can not be raised unless SWP /SOP is available and site
induction is given based on the same.
Return of Permit

• This has to be done/signed off only after completion of job, whereas removal
of all materials / men from site has done by Working Agency.
• Hence now Equipment & Area is safe for operation also a the used
Equipment / Facility can be restored.

Procedure
S1.Initiator will not start the job while permit is under approval, but can do the
preparatory job till permit gets approved from Issuer. The Initiator shall ensure that
the persons engaged are thoroughly briefed about the relevant SOPs / SWPs,
outcome of Risk Assessment & its mitigation. He will also ensure that relevant PPEs
are provided. He shall ensure and fill “Initiator’s check List”. He will sign the permit.
S2.Initiator shall ensure and explain to the work group about initiator’s check list,
hot work check list and control measures stated in over leaf of the permit safe
executing the job. Initiator together with the Co-permittee shall jointly inspect the
site to confirm that all preparation work is completed. He shall sign in permit and
obtain signature of co-permittee & hand over the permit to the workgroup for display
at job site.
S3.Dully signed work permit shall be kept at site where work group is performing
the job.
S4.Initiator is responsible to maintain the status-co for control measures stated in the
permit during execution of work safely.
S5.Co-permittee is equally responsible to maintain the status-co for control
measures stated in the permit during execution of work safely.
Closure of Integrated Work Permit
(Cold & Hot )

S1. When the job is completed, the Initiator shall ensure that all men
and material are removed from the site and ensure that isolations
made, is restored. He shall obtain signature of the co-permittee
and sign off the permit. He shall hand over the Permit copy to
Initiator to close the permit.

S2. Initiator shall file the permit in ‘completed work’ file as record
for 7 days unless the job involves any incident/ business
implications..
Distribution of Permits
• First copy of the work permit & SWP /SOP shall be with
initiator / co-permittee and shall be at job site.

• The second copy shall be with issuer and shall be displayed at


issuer’s office/control room.

• After completion of work, 1st copy of the permit shall be filed by


initiator in a "Completed Work" file. Issuer shall file 2nd copy of
permit in his “completed work “file.

• Both initiator and issuer shall retain these copies as record for 7
days.
Renewal of
Integrated Work Permit
• Permit shall remain valid for the duration of maximum one day.
Day starts at the start of General shift.
• All previous day(s) work permits need to be renewed at the start of
General Shift.
• While permit is due for renewal and / or permit is under renewal
process, Initiator shall not start working on the job.
• Work permits need to be renewed…..
a. At start of General Shift every day.
b. Whenever initiator is changed for any reason.
c. Whenever Co-Permittee is changed for any reason
Renewal of
Integrated Work Permit
S1. Initiator shall brief risk assessment and its mitigation to work
group and obtain signature of Co-permittee.

S2. Issuer shall sign in 2nd copy of permit with him and shall also
sign 1st copy and return 1st copy to initiator.

S3. Initiator shall restart work after renewal by issuer.

S4. In the event of change in work group, any addition of person(s)


in the work group, Renewal of Permit from issuer is not required.
However Initiator should explain safety measures, risk assessment
and its mitigation to new person(s) in the work group and shall sign
and shall obtain signature of co-permittee
