Requirements: - Complete signed Transfer of Credit Form (TC Form) 3) Magpa-pirma sa mga sumusunod: - Transcript of Record (TOR) - Pumunta sa Dean ng/ng mga department na ipapa-credit nyo na - Money for fee subject. - Valid ID (need to present to guard para makapasok sa campus) - After magpa-sign sa mga Dean, pumunta sa Institute of Open and -Screenshot ng pic ng Proof ng Appointment Distance Education (IODE) Director (Room E-309) at tsaka sa PUP OUS Executive Director (Room E-304A) para magpa-sign rin. 1. Set appoinment sa PUP Appointment System βeta (https://apps.pup.edu.ph/appointment/) para makapasok sa campus. 4) After mag-sign ay pumunta sa Accounting Office sa may South Wing para computation ng babayaran. - +Request Appointment - Avail PUP Services (ex. Pay Fees, Claim Documents, etc,) 5) After mabigay ang computation from Accounting ay pumunta at - Appointment Information pumila sa Cashier para magbayad. - College / Office: OUS – Open University System Registrar - Type of Service: Submission of Entrance Requirements/Accomplished 6) Ipa-xerox yung completed signed na TC Form (tig 2 pieces tsaka Completion Form/Transfer of Credit/Re-admission Form / Cross yung resibo kung gusto since kukunin yung original na resibo ng OU Enrollment/ Change of Enrollment/ Certificate of Transfer Credential and Registrar) at ng inyong TOR. other documents - Date of Appointment 7) Pumunta sa Open University System Registrar para ipasa yung TC - Time of Appointment Form (original and xerox) at xerox ng TOR. Bibigyan kayo ng copy ng signed na form para ipakita sa professor ng subject na i-credit. 2) Fill-up-an yung Transfer of Credit Form.
