Introduction to Psychosis
 Definition: Derived from Greek words
'physche' (mind) and 'osis' (disorder).
 Psychosis is a mental health condition

characterized by a disconnection from


reality.
Symptoms:
 - Hallucinations
 - Delusions
 - Disorganized thinking
 - Disorganized behavior
Antipsychotic Drugs

 Definition: Medications used to manage


symptoms of psychosis and related disorders.
 Indications:

- Schizophrenia
- Bipolar disorder
- Severe depression
- Anxiety disorders
 Effects: Regulate thought patterns, mood

stability, emotional responses, and behavioral


control.
Mechanism of Action

 Typical Antipsychotics:
 - Block dopamine D2 receptors in the brain.
 - Reduce positive symptoms of psychosis (e.g.,
hallucinations, delusions).

 Atypical Antipsychotics:
 - Block dopamine D2 and serotonin 5-HT2A
receptors.
 - Target both positive and negative symptoms of
psychosis.
Classification of Antipsychotics

 1. Typical (First-Generation).
 - MOA: Block D2 receptors.
 2. Atypical (Second-Generation)
 - MOA: Block D2 and 5-HT2A receptors.
Typical Antipsychotics

 Examples: Haloperidol, Fluphenazine,


chlorpromazine, perphenazine
 Adverse effect;Extrapyramidam, neurolepctic
malignant syndrome
 Contraindication; coma and severe nervous
system depression ,parkinsons’ disease,
hypersensitivity
 Indications: Schizophrenia, severe agitation.
 Side Effects: dry mouth, drowsiness, dizziness,
sedation, constipation, weight gain
Atypical Antipsychotics

 Examples:Risperidone,Clozapine,Olanzapine,Quetiapin
e.
 Adverse effect: metabolism syndrome agranulocytosis
and cardiovascular events
 Indications: Schizophrenia, bipolar disorder,severe
depresssion
 Contraidication :severe liver and kidney disease,
hypersentivity, pregnacy and breast feeding

 Side Effects: Weight gain, sedation, increase in


appetite , gastrointestestinal issue, erectile disfuntion
and decrease libido , amenorrhea and dysamenorrhea
Pharmacokinetic Profiles
 Drug 1: Haloperidol (Typical Antipsychotic)
 - Bioavailability (F): 60-70%
 - Maximum concentration (Cmax): 4-8 ng/ml
 - Time to reach Cmax (T.Cmax): 2-6 hours
 - Half-life (T½): 24 hours
 - Volume of distribution (Vd): 14-24 L/kg
 - Clearance (Cl): 0.5-1.5 L/h/kg

 Drug 2: Carbamazepine (Mood Stabilizer)


 - Bioavailability (F): 75-85%
 - Maximum concentration (Cmax): 4-8 ng/ml
 - Time to peak (Tmax): 4-8 hours
 - Half-life (t1/2): 25-65 hours
 - Volume of distribution (Vd): 0.8-1.9 L/kg
 - Clearance (Cl): 0.2-0.6 L/h/kg
