A formalized root cause analysis process is used consistently to identify root causes of incidents,
near misses, or equipment failures as appropriate.
The incident investigation and root cause analysis team(s) include the appropriate mix of
personnel (or ready access to personnel) with knowledge of equipment, operations, human
performance, engineering and/or other pertinent areas.
All operations personnel have received introductory level training in incident investigation and
root cause analysis; supervisors, analysis team leaders, and/or others as appropriate have received
more advanced training.
Self-reporting of near misses, errors, or other performance problems is encouraged, free of
punitive action, and routinely practiced by personnel.
The root cause analysis process produces recommendations for improvements to eliminate
systemic problems; those recommendations are evaluated and acted upon (approved or rejected
with good cause) promptly.
Responsibilities for action in response to root cause analysis are clearly defined, and
improvement actions are tracked and monitored for effectiveness.

Lessons Learned
A systematic lessons-learned program is in place to capture and assess information on successful
and unsuccessful performance, develop applicable lessons learned to improve safety and
effectiveness, and disseminate lessons learned to all applicable personnel.
The lessons-learned program collects and evaluates a diverse and comprehensive array of source
data (e.g., root cause analysis, near-miss reports, equipment failure histories, customer survey
data, performance measures, drill critiques, industry reports, problem-identification reports,
production data, etc.)
Lessons learned are summarized effectively and disseminated to appropriate personnel in a timely
manner.
Follow-up monitoring is conducted to assure implementation and assess effectiveness of
recommendations from the lessons-learned program.
The overall effectiveness of the lessons learned program is measured using both process and
outcome indicators.

Operational Performance Feedback Rating Sheet No/Strongly Disagree 0 Disagree 1 Agree 2 Strongly Agree 3 Question Incident Investigation Score (0 to 3) A formalized root cause analysis process is used consistently to identify root causes of incidents, near misses, or equipment failures as appropriate. The incident investigation and root cause analysis team(s) include the appropriate mix of personnel (or ready access to personnel) with knowledge of equipment, operations, human performance, engineering and/or other pertinent areas. All operations personnel have received introductory level training in incident investigation and root cause analysis; supervisors, analysis team leaders, and/or others as appropriate have received more advanced training. Self-reporting of near misses, errors, or other performance problems is encouraged, free of punitive action, and routinely practiced by personnel. The root cause analysis process produces recommendations for improvements to eliminate systemic problems; those recommendations are evaluated and acted upon (approved or rejected with good cause) promptly. Responsibilities for action in response to root cause analysis are clearly defined, and improvement actions are tracked and monitored for effectiveness. Lessons Learned A systematic lessons-learned program is in place to capture and assess information on successful and unsuccessful performance, develop applicable lessons learned to improve safety and effectiveness, and disseminate lessons learned to all applicable personnel. The lessons-learned program collects and evaluates a diverse and comprehensive array of source data (e.g., root cause analysis, near-miss reports, equipment failure histories, customer survey data, performance measures, drill critiques, industry reports, problem-identification reports, production data, etc.) Lessons learned are summarized effectively and disseminated to appropriate personnel in a timely manner. Follow-up monitoring is conducted to assure implementation and assess effectiveness of recommendations from the lessons-learned program. The overall effectiveness of the lessons learned program is measured using both process and outcome indicators. Total Scoring: 28 - 33………….Op Feedback system is well designed.
