Academic WorldQuest 2011 Dinner Registration Form Dear Parents and Guests of our Academic WorldQuest Participants, Please join us for Academic WorldQuest 2011 on Tuesday, November 15 from 5:30-9:00 p.m. at the University of North Florida’s University Center. Academic WorldQuest is a fast-paced and competitive event where teams of 4 students compete in six rounds of trivia concerning world affairs and global issues. The World Affairs Council of Jacksonville will sponsor the winning team and their teacher at the national competition in Washington D.C. on April 21, 2012. In an effort to thank our student and teacher participants as well as our Council volunteers, we provide a complimentary dinner prior to the event. Unfortunately, our funding is limited and we can only provide a limited amount of complimentary meals which are reserved for our students, teachers and volunteers. However, if you would like to join us for dinner prior to the event, we are glad to accommodate you and your family members at a cost of $15.00 per person, which simply covers the cost of a meal. There is no profit made by the Council as a result of this purchase. Purchasing dinner at $15.00 per person includes: garden salad, entrée (e.g. pasta, etc.), vegetables, rolls & butter, assorted cookies, and ice tea and/or water. If you would like to purchase a meal ticket, please call the Council at 904-280-8162 or cut off the form below and mail it in with your credit card information or a check made payable to the World Affairs Council of Jacksonville (3948 Third Street South #380, Jacksonville Beach, FL 32250.) We accept all major credit cards. Tickets must be purchased prior to November 4, 2011. Accommodations CANNOT be made the night of the event and MUST be purchased prior. If you choose to purchase a ticket by November 4, your meal ticket(s) will be waiting for you the night of the event.
