SKILL PERFORMANCE CHECKLIST
INTRAVENOUS INFUSION

I ASSESSMENT
1 Identify the patient
2 Check physicians order indicating type of solution,
amount to be administered, rate of flow, etc.
3 Assess the Vital signs, Skin turgor, Allergy to latex,
tape, or iodine, bleeding tendencies, disease or injury
to extremities, status of veins.
II PLANNING
1 Gather necessary articles
2 Place the patient in comfortable position
3 Prepare the environment for the procedure.
III IMPLEMENTATION
1 Perform hand washing
2 Open and prepare the infusion set. Remove tubing
from the container and straighten it out. Close the
clamp and leave the ends of the tubing covered.
3 Remove the protective cover from the entry site of
the bag. Remove the cap from the spike and insert the
spike into the insertion site of the bag or bottle
4 Apply a timing label on the solution container.
5 Hang the solution container on the pole
6 Partially fill the drip chamber with solution
7 Prime the tubing.
8 Perform hand hygiene again just prior to client
contact and put on the gloves
9 Select the venipuncture site.
10 Place a mackintosh under the arm
11 Dilate the vein. Apply a tourniquet firmly 15 to 20
cm (6 to 8 in.) above the venipuncture site.
12 Clean the site using circular motion.
13 Use the non dominant hand to pull the skin taut
below the entry site.
14 Holding the over-the-needle catheter at a 15- to 30-
degree angle with bevel up, insert the catheter
through the skin and into the vein.
15 Once blood appears in the lumen of the needle or feel
the lack of resistance, lower the angle of the catheter
until it is almost parallel with the skin, and advance
the needle and catheter approximately 0.5 to 1 cm
(about 1/4 in.) farther.
16 Pull the needle little back wards, at the same time
insert the catheter till hub touches the skin.
17 Release the tourniquet.
18 Tape the catheter
17 Press the area of skin just above the insertion site and
remove the needle.
18 Start the infusion and ensure appropriate infusion
flow.
19 Dress and label the venipuncture site and tubing
20 Document the procedure
21 Discard the waste
IV EVALUATION
1 Evaluate skin status at IV site
2 Evaluate status of dressing
3 Evaluate Ability to perform self-care activities;
understanding of any mobility limitations
4 Evaluate vital signs compared to baseline level
